School Immunization Requirements – Indiana
Information from the Clark County Health Department about required vaccinations for students entering Kindergarten, Grade 6, or Grade 12.
Kindergarten Requirements
All students entering Kindergarten in Indiana are required to have received the following vaccines prior to the first day of school:
- 5 DTaP
- 4 IPV
- 3 Hepatitis B
- 2 Hepatitis A
- 2 MMR
- 2 Varicella
6th Grade Requirements
Students entering 6th grade must have all Kindergarten vaccines, plus:
- 1 MCV4 (Meningococcal)
- 1 Tdap (Tetanus, Diphtheria, and Pertussis)
ACIP recommends the HPV vaccine series beginning at age 9. This is a good time to start if not already completed.
12th Grade Requirements
Students entering 12th grade must have all previous vaccines, plus:
- 1 MCV4 (2nd dose – Meningococcal)
