Death Records

The Clark County Health Department maintains death records for individuals who have died in Clark County, Indiana only.  If you are an immediate family member or have a vested interest in obtaining a certified death certificate or a copy of a death certificate please contact the registrar at (812) 282-752 to determine if a certified copy can be provided. 

For your convenience the form for obtaining a certified death certificate can be completed and printed in advance and submitted to the Clark County Health Department.  This record may be available immediately by visiting the office if all information is correct and you have confirmed vested interest or relationship for the document.

What If I Don't Live In The Area?

If you currently live in a different state or cannot visit the health department in person you may request a certified death certificate by doing the following:

To request a copy by regular mail please include the following:

• A self-addressed, stamped envelope to the
            Clark County Health Dept.
            1320 Duncan Avenue
            Jeffersonville, Indiana  47130

• A completed certified death certificate

• Copy of Current Driver's License or other required documentation

• Include the appropriate fee(s) in the form of a check or money order to the Clark County Health Department
